ROLE OF UGI ENDOSCOPY IN PEDIATRIC POPULATION

Since its introduction in the 1960s, the field of Pediatric Gastroenterology has developed rapidly worldwide.  The number of Pediatric Gastroenterology centers in India have increased in last two decades.

What is different in Pediatric Endoscopy

Being a Pediatrician/Pediatric Gastroenterologist requires special skills and experience in dealing with children. A great portion of empathy, patience, and psychological skills are required. Before endoscopy, a detailed pediatric specific history and examination has to be done. During the procedure less air is to be insufflated. Excess air can lead to a lung compression in infants and can lead to worsening of the respiratory status. CO2 insuation instead of air can reduce such problems as it is much faster absorbed and exhaled. In contrast to endoscopy in adults, in children it is preferred to take biopsies from all relevant areas, even if there is no visible pathology.

Indications of UGI Endoscopy in children

Diagnostic Indications: Therapy refractory GER; Corrosive injury; Peptic ulcer disease; Chronic diarrhea/Impaired growth; IBD – Crohns Disease; Variceal bleeding; Gastric/Duodenal ulcer bleed; Food allergy/ Eosinophillic esophagitis; Esophageal candidiasis.

Therapeutic Indications: Esophageal/Pyloric stricture dilatation; Foreign body removal; Percutaneous Endoscopic Gastrostomy (PEG); Congenital webs; Achlasia
