In Alagille syndrome, xanthomas, or fatty deposits under the skin, are caused by chronic cholestasis, a condition in which the bile ducts do not properly drain bile from the liver. This buildup of bile leads to increased levels of cholesterol in the bloodstream, which then accumulates in the skin.
